This form is only for faculty & staff within the School of Arts & Sciences. Your request will be processed after confirmation from your department. We will send your account information to you at the campus box provided on this for form after the account has been activated.
Accounts for research labs, course accounts, and other administrative accounts are handled on a case-by-case basis. Each non-individual account request must include the name of a staff or faculty member who will be responsible for the activity on the account. For security purposes, accounts where the passwords are expected to be shared among a number of individuals are not created. More information can be found here.
